Add Gender to the main student enrollment form

WIth so many gender-neutral names for kids nowadays, it's hard to know whether a student is male or female. I added a Gender field to my Additional Data form, but it is such a basic question asked on all questionnaires that it really should be on the main form as a required field. WIthin that could be the other options, including they, etc.
