Have parents sign a participation form while putting in costume size

We have a participation form that all the parents have to sign before putting in their costume size. It would be nice if that could be an option in the costume portal so parents could do it all at once.

Also if they could choose per class if they wanted to participate or not. A little checkbox for participation and to sign a waiver would make life so much easier for our parents.
