Remove waiver check box once checked

Hello!

Once a parent checks the waiver box and agrees to their waiver it shows the date they checked it but it's confusing as it then remains on screen with the box unchecked! If you don't know what you're looking at it, t looks as though it hasn't been signed (if they don't see the text!) which could lead to parents potentially sign it over and over!

Is there a way the system can say an automatic 'Thank You' type of messaged then a '"Your waiver was signed on....." type of idea and remove the check box option once the box is checked?
