Add a place for parents to write in any health issues of dancers when registering online

In the Parent Portal, there is a place to "Ask for Medical Detail" to check off medications, Special Needs, Allergies and Primary Dr. I would just want a box where WE could write something like this and have a box that they can type in details... "please explain any health conditions that we should be aware of: For example, allergies, anemic, diabetes ADD, Low muscle tone, anxiety, special needs, selectively mute, hearing issues, eyesight issues, person who stutters, and please share any tips to help us to give your student the best experience we can." Then a box where they can give us details.

I am seeing this more in my studio and I see other studio owners complaining on social media that some parents "forget to tell us" some important things like the things listed above that we should know to help our students. If your autistic child doesn't like to be touched, you need to tell us or when we go to hold hands in a circle or the teachers tries to fix her tap shoes and she has a meltdown, it would be good information to know ahead of time to prevent a meltdown. (That one happened to me! After me asking Mom, is she ok with loud music, Mom said yes, I asked anything else I need to know, she said no. BUT not being touched was kind of important thing to know!!!) Thank you!

Plus customizations to this section! So that we can ask in a way that is conscious of how they identify. Saying "special needs" is no longer acceptable to the community and some people identify as disabled but other identify as having a disability. We need a way to ask these questions and have the ability to change the field as language changes in these communities.
