Current waiver does not list the SPECIFIC STUDENT NAME, just the parent.

TO avoide potential LIABILITY ISSUES from not naming student on signed waivers - Current waiver does not have a place for parent to write the SPECIFIC CHILD'S name - they should actually have a form for EACH CHILD in family and their name WRITTEN OUT (could easily add this where parent writes their name at the bottom of the waiver. They are not agreeing to THAT child on waiver, need to specify WHO they are making agreement for!
