I think it would be helpful if parents have the option to choose non-binary (if they know their child identifies as non-binary). It would also be helpful to know if there is a preferred name and/or preferred pronouns.
Could it be left as parents needing to choose male or female, but having a checkbox after that for if their dancer identifies as non-binary? It is important for us as studios to know both how the dancer identifies and what we need to tell medical personnel in case of emergency.
