Change the box sizes in the class sign-in report

The class sign in sheets (reports) are currently useless when printed as it makes too many pages to print.

Could you increase the size of the Name box so full name is on one line in the print out.

Shorten the In and Out boxes by about 60% each.

Most classes should now print on one page instead of two.
