Separate Address Information Merged

It would be great if the address information was in separate fields. Address/Apt-Suite/City/State/Zip. This would ensure that we receive all the necessary information. Also, the use of dashes when putting in phone numbers, at times the numbers are all together making it difficult to read/verify.
