Hello, we are finding that people do not complete all the information required. I'd like to suggest that you make the field's mandatory for the following; Mother's name, tel #, email address, father's name or alternate contact, tel#, email address, mailing address including town, province/state and postal code/zip code.
It would also be great if they couldn't put the same name, phone number and email address is both the mother and father fields. Thank you, A. Tucker
This would be so helpful! Whenever I look at the "how did you hear about us" box, it always just says "online registration" because no one takes the time to fill it out. This would be an amazing resource for collecting data but unfortunately goes unused because it's not mandatory. Also, I would agree with you about making all other information mandatory. One time someone didn't provide a phone number and we had no way to contact them to pick up their sick child.
Adding to that perhaps the option to choose which fields you'd prefer to be mandatory.
