Waivers for drop in students

A form we could load up on an ipad or computer for students to sign a waiver with or without making an account. We often have students visiting town who come to an open gym or drop into a single class who need to sign a waiver but, don't need to go through the process of making an account. It would be efficient to have them input all of their information on an ipad or computer that we set up near the front desk. Ideally there would be the option for them to create an account after putting in their information as well.
